MAPP stands for “Mobilizing for Action through Planning and Partnerships.

MAPP Toolkit

The Maine MAPP Toolkit (PDF* 32MB) was developed by the Division of Local Public Health/Maine CDC with funding from several federal funding sources.   It is adapted from elements of NACCHO’s “Achieving Healthier Communities through MAPP: A User’s Handbook”, originally funded by the Public Health Practice Office of the US Center for Disease Control and Prevention (CDC).

MAPP uses a self-assessment approach and reflects the practice needs of Maine’s Healthy Maine Partnerships who are implementing the MAPP process to develop Community Health Improvement Plans.
