District Public Health Improvement Plans
District Public Health Improvement Plans (DPHIP) are developed by the District Coordinating Councils for Public Health. Their purpose is to address specific and unique strengths and health needs of all the communities within each District. During the next two years, the Plans will be used to improve public health service coordination, address system issues and gaps, and reduce the rate of avoidable hospitalizations as called for in the 2010 State Health Plan.
