Resources For Reporting Physicians

The following reports are available for download in either PDF or Microsoft Word format. You will need the free Adobe Acrobat Reader to view and print PDF files. Microsoft Word format requires either Microsoft Word or the free Microsoft Word Reader.

Requirements:

According to state law, physicians are required to report any newly diagnosed cancer case when the patient will not be referred to a hospital for diagnosis or treatment. Currently, MCR receives paper reports from physician offices using the following forms and reporting instructions.

Confidentiality:

Because MCR is a Public Health Authority, reporting to the MCR is allowed under HIPAA regulations. The following document addresses some of the frequently asked questions about HIPAA and reporting patient information to the cancer registry.

Future Plans:

With the advancement of new technology, national goals have been established to promote electronic reporting of cancer cases to central cancer registries. MCR hopes to have the resources to facilitate secure web-based reporting within the near future.
