H1N1 Vaccine Doses Administered Aggregate Counts Weekly Reporting Form

Reporting periods start on Sunday and end on Saturday of each week.  Regardless of the days you actually administer vaccine during the seven day period you are reporting, please ensure that your Report Start Date falls on the Sunday of that report week and your Report End Date falls on the Saturday of that report week.   Reports must be submitted by close of business on Saturday of each week that doses have been administered.  It is not necessary to submit a form during weeks your facility has not administered H1N1 doses.
