Immunization Registry

Now for the first time ever, individual shot records can be housed in one safe, secure location.

The Maine Immunization Information System (IIS) is managed and maintained by the Maine Immunization Program, an agency of the Maine Centers for Disease Control and Prevention. The majority of shot records are entered by private providers like your family doctor.
Now called ImmPact2 the Maine IIS has grown over the years and holds thousands of shot records, but not all providers are participating.
Other than your medical chart, ImmPact2 is the only system authorized by statute to collect, store and maintain immunization records for Maine citizens.

Although the focus has been to preserve children's shot records ImmPact2 holds many adolescent and adult records.

Ask your provider if they participate in ImmPact2.

How do I access my or my children's shot records?

1. Contact your health care provider, if your records have been transferred into ImmPact2 they can give you an instant official shot record printout.
2. If your provider does not participate in ImmPact2 they should still have a record of vaccinations in the paper chart and can make you a copy.

What if my or my children's shot record is missing?

1. If you have moved check with previous doctors office to see if they have archived your records
2. Check with your school to see if school records exist.
3. Send a notarized letter affirming your identification to the Maine Immunization Program:

   Make sure you include the first name, last name and date of birth of all who's records are needed. We will search ImmPact2 and send the records to you if found.
