Simulation Name: Reading Assessment Information
Simulation Mode: Show me
Scenario: In this lesson, you will be shown how to view assessment information. Think of assessments in EIS as paper forms that you might have had in a client's file.
Screen Name: #1: People Demographics - Microsoft Internet Explorer
Step: Click ASSESSMENTS in the EIS Menu "Hot Area"  |
| User Prompt: You have already anchored your person. Click ASSESSMENTS in the EIS Menu to see their assessments. |

Screen Name: #2: Assessment List - Microsoft Internet Explorer
Step: Click Assessment ID "Hot Area"  |
| User Prompt: To view an assessment, click the underlined Assessment ID (example: MR Services and Supports Assessment). |

Screen Name: #3: Assessment General - Microsoft Internet Explorer
| Step: Select Questionnaire from the Assessment SHOW menu drop-down list. |
| User Prompt: This is the Assessment General page. It is the same for any type of assessment. Think of this page as a "Cover Sheet" for the assessment that has basic information. To view the actual assessment, select "Questionnaire" from the SHOW menu. |

| Step: Select * Potential Service and Support Needs from the DIMENSION drop-down drop-down list. |
| User Prompt: You are now viewing the first page of this assessment (some assessments have just one page, while others have multiple pages). To view the other potential pages of an assessment you must select them from the DIMENSION drop-down box. For this assessment, we'll select "* Potential Service and Support Needs" from the DIMENSION drop-down box. |

| User Prompt: Since this particular assessment has a third page, let's select "* Unmet Needs" from the DIMENSION drop-down box. |

| Step: Select Notes from the Assessment SHOW menu drop-down list. |
| User Prompt: Some assessments have notes attached to them. To see if an assessment has a note attached, select "Notes" from the SHOW menu. |

Step: Click underlined note TITLE "Hot Area"  |
| User Prompt: To view the actual note (if there is one listed), click the underlined note TITLE. |

Screen Name: #9: Notes General - Microsoft Internet Explorer
Step: Click ASSESSMENT in the EIS Header "Hot Area"  |
| User Prompt: Read the note information. To go back to the assessment, click ASSESSMENT in the EIS Header. |
