Dear Potential Applicant for Project Funds:

I am pleased to send you the enclosed information about an exciting opportunity to participate in a grant project. Enclosed you will find an invitation to submit a letter of interest for pilot project grant funds as part of a Real Choice Systems Change Grant from the Centers for Medicare and Medicaid Services.

We have assembled a small working group to function as the Resource Team for the project and we are eager to work with interested parties to develop pilot project ideas.We have attempted to make the project application process simple and straightforward.It is our goal to encourage creative and cooperative ideas and provide assistance throughout the project development and implementation.

To that end, please review the information in this invitation and consider attending aninformational meeting at the Department of Human Services Building, 442 Civic Center Drive, Augusta, Conference Room 1A, on June 15^th from 3 – 5 pm.The Resource Team will be available to encourage an exchange of ideas among all interested parties and to answer your questions. We would appreciate an RSVP as we plan to provide refreshments for the attendees.
