SUBJECT:	Med 3.0 version and revised MED 3.0 Instruction Manual

 

Attached is the Medical Eligibility Determination (MED) form version 3.0 and the outline of the changes, additions and revisions in the tool.  Begin using the MED 3.0 version immediately upon receipt.  Be sure to destroy any prior versions of the MED tool as they are no longer valid and will not be accepted for reassessments submitted to the Department for reclassification of medical eligibility as August 1, 2001.

 

For Home Health providers who complete the form for Section 96 of the MMAM for recipients under the age of 21, and for the medication and venipuncture only services for those over the age of 21, a supply will be available at BMS. In the meantime several copies have been included that can be used to make copies as needed.
