1. When will the Support Waiver begin?
a. October 1,2007.
 
2. Who needs to go onto the Support Waiver?
a. People who are presently using Free Standing Day Habilitation, grant dollars for Supported Employment, grant dollars for Sheltered Employment if they are going to convert to other areas of support that are in this waiver, and people who are on the family support program.
 
3. Who may not be eligible at this time?
a. People in nursing homes who are in Free Standing Day Hab, people who are not on Medicaid, and people who are on another waiver program such as the Adults with Disabilities waiver. People in nursing homes will stay on Section 24 until we can find a mechanism to fund. People not on Medicaid because of income guidelines should look to reapply as the ceiling is increased with a waiver. Please see your Supervisor or a Resource Coordinator for this assistance. (Note: This last statement would not pertain to people receiving Community Case Management.)
 
4. How do I make application to the Support Waiver?
a. Refer to the documents on this site.
 
5. What is the change for someone moving from Day Habilitation to Community Support?
a. There doesn’t need to be a change unless the person chooses to have one. Community Supports allows more flexibility (see fact sheets on this site) in programming than day habilitation.
 
6. How much time can a person spend in Community Supports?
a. A person has access to up to 1300 hours of support if needed and identified in the Person Centered Plan.
 
7. Can I get employment support on the Supports Waiver?
a. - The Support Waiver provides up to 300 hours of employment support a year. Any time used is subtracted from the 1300 total hours available. Please refer to the fact sheet on employment services for more information.
 
8. Is case management required and who provides it?
a. It will be a requirement that person has case management on this program. At a minimum the person will need to agree to assistance with planning, maintaining waiver eligibility, and two program visits a year. Adult Community Case Management and State Case Management will provide the services. A person between the ages of 18-21 who wants to recieve this service has to come into adult case management.
 
9. Can respite pay for Camp?
a. No
