Description of Services:
Youth & Family Services, Inc. is a Non-Profit Organization serving youth and families. We have provided case management and family services to individuals and their families in Central Maine (Kennebec and Somerset counties) since 1974 and were one of the first agencies in the state to provide Children’s Mental Health Case Management. Our staff is very knowledgeable and experienced in local services, supports and providers. These are provided out of our Skowhegan and Augusta offices.

Our Programs include:

  • MR Community Case Management, Outpatient therapy, Substance abuse services, Family therapy,
  • Homeless youth outreach, Family support program, Children’s MH/ MR/ Autism Case management,
  • Emergency youth shelter

Adult MR Community Case Management Program Description:
Case Managers work with Adult MR clients ages 18 and up, their families or guardians, and other providers to develop and implement “Person Centered Plans (PCP’s)”. These plans identify needs in areas such as day habilitation, guardianship, transportation, job development, supportive employment, recreational and social activities, residential concerns, therapeutic and medical needs, in home supports, Special Olympics and other needs/desires. Goals and objectives are set to help meet these needs. Case Managers make referrals to needed services, and advocate for consumers. Case Managers don’t provide these services directly. Clients served are those adults, 18 and up, who have a MR diagnosis, have been found eligible for Adult MR services by DHHS and who reside in Kennebec or Somerset County. The Program is staffed by Case Managers with a minimum of LSW licensure, supervised by Master’s level staff. Services are funded by DHHS/Mainecare.

Community Case Managers meet with Individuals in home, school, office and community settings once a month to develop and monitor service plans and meet goals. The case manager will call the client, family, guardian or collateral contact (other service providers) weekly. Case Managers work with clients, family, and providers to assist and advocate for the best services possible. Service continues until goals are met, client elects to stop, individuals are non-compliant with program requirements, client no longer lives in the service area, or client returns to state case management due to waiver services.
