  • January 25, 2008
    The Team Recommendations Section 21 form has been corrected. On the previous version, Counseling was listed under Consultation, which was an error.
  • December 2007 New Forms to be used as of January 1, 2008 or sooner: Revised Face Sheet, New forms for Team Recommendations for waivers (Sections 21 and 29), and Guardian Approval form. See PCP Form Instructions for detailed instructions for each of these forms.

    New PCP Data Collection Forms are to be used beginning January 2007
  • September 12, 2005
    The Face Sheet and the Notification/Attendance forms have been revised. Dates on the Desire/Need, Continuing Services, and MR Interim Plan forms have been changed to MM/DD/YY format. the Instructions have been updated to conform with the revised forms. Please download these revised forms and begin using them by October 1, 2005.

    Face Sheet, Continuing Services, Notification and Attendance, and Response Sheet forms and their instructions have been updated. Please download and use the new versions
  • February 19, 2004
    We have been asked NOT to include consumer's name or initials in Plan Name on the Face Sheet. From now on use a generic name, such as Annual 04 or Interim Employment Feb04. This is a change from earlier instructions and training. Apparently names and initials can appear in some reports generated by the database and could be viewed by people who should not have access to all names. Names and initials on plans already received are being deleted, so you do not need to do anything to correct those plans.

    Need/Desire Sheet - the Domains space has been eliminated

    Need/Desire, Continuing Services, Notification, Interim Plan - all have a space at the top for consumer initials so that if a page gets separated it can be re-attached to the right plan

  • Dec. 10, 2003:
    Action Plan Face Sheet - There is now has a space for Consumer MaineCare ID number. Please start using the updated form and enter the MaineCare ID as well as the Social Security number.

    Face Sheet - If an unmet need is identified at the time of the Annual Plan you may check both "Annual" and "Interim" under Plan Type on the Face Sheet and include the Interim plan with the Annual. An Interim Plan written any other time during the year needs its own face sheet.

    Need-Desire - You may leave the Domains space blank.
