DDPC Immunization Requirements

Mandatory
- MMR (Measles, Mumps Rubella)
- If born before January 1, 1957 no proof of immunization is needed.
- If born after January 1, 1957 you must show proof of 2 MMR immunizations
or
- Provide proof of positive MMR titer (blood test) indicating immunity.
- Varicella (Chicken Pox)
- Evidence of 2 varicella immunizations
or
- Positive varicella titer indicating immunity
or
- Document signed by employee stating “to the
best of their knowledge”, the month/year they
were infected with Chicken Pox.
- Tuberculin Skin Test (also known as TST or PPD)
- Documentation of a “two-step” TST within the past 12 months
- Documentation of a negative TST within the past 12 months followed by a 1-step TST at DDPC prior
to orientation (for new employees/volunteers only. Students are responsible for meeting
healthcare immunization requirements prior to clinicals.)
- New employees/volunteers who have no TST history within past 12 months must contact
DDPC Employee Health to schedule a 2-step TST at least 2 weeks prior to start date.
or
- If past TST test was positive, provide documentation of the positive TB test and proof of a follow-up
negative chest X-ray.
- All students must meet immunization requirements prior to starting clinical assignments.
- Students must provide immunizations documentation.
Recommended
- Hepatitis B
- If you have received the 3-part Hepatitis B immunization series, please provide documentation of the
series.
- The 3-part Hepatitis B immunization series is highly recommended (OSHA). It is offered to all
employees/volunteers at risk of exposure to blood borne pathogens at no cost.
- If the Hepatitis B series is declined, DDPC requires the employee/volunteer to sign a Hepatitis B
Declination Form.
- The Hepatitis B series will be available at no cost to all employees/volunteers who initially decline
the Hepatitis B series.
