Updated May 2008 - Removing Underground Storage Tanks Or Underground Product Piping - Summary of notification requirements to the Department before and after the tank/product piping excavation.

Removal Notice (MS Word Format) (pdf format) - This form must be filed with the DEP and your local fire department at least 30 days prior to the scheduled removal. Submit the Removal Confirmation (page 2 of the form) after the tanks and/or piping are removed.

Site Assessor’s List - (pdf format) - Site assessors available to perform a site assessment during underground oil storage tank removal. A site assessment must be conducted when any tank used for storage of motor fuel (gasoline, aviation fuel or diesel), heavy oil, waste oil or used for marketing and distribution, is removed or abandoned in place.
