Retrofits
Facility Upgrade Form- Notify the Department of piping and tank upgrades. File with the Department 10 business days before the work begins.
Spill Bucket Dimensions - Upon replacement of a spill bucket, if the required 15-gallon spill bucket will not fit, the largest capacity must be used. The tank installer must submit justification to DEP as to why a smaller spill bucket is being used. Out of Service Tanks
Requirements for Temporarily Out-of-Service Tanks – Requirements that must be met in order to take your tank out of service, and return your tank to service.
Notification of Temporarily Out-Of-Service Tank(s) Form – If you are taking your tank out-of-service, you must complete and submit this notification form to the Department.
Notification of Return to Service Form – If you are returning your tank to service, you must complete and submit this notification form to the Department.
Change of Ownership
Change of Ownership Form (MS Word) (pdf format) - A change in facility ownership requires the new owner or operator to amend the facility registration within 10 business days of the transfer of ownership. Use this form to meet Department notification requirements.
